Abstract
Background and Aims: The International Consortium of Optic neuritis (ICON) has published consensus diagnostic criteria for optic neuritis in 2022. This criteria facilitate an accurate early diagnosis which is crucial for timely initiation of treatment. In addition there is a consensus classification of optic neuritis. This guides the further management and choice of treatment for conditions like multiple sclerosis, neuromyelitis optica spectrum disease, and myelin oligodendrocyte glycoprotein antibody disease. / Methods: A 3 year Delphi process based on real cases from around the world. Consensus was accepted for at least an 80% level of agreement between the ICON experts. / Results: The ICON 2022 diagnostic criteria distinguish between possible and definite optic neuritis, offering a systematic approach for accurate classification. The initial diagnosis relies on clinical features for possible optic neuritis, with subsequent para-clinical tests employing brain, orbital, and retinal imaging, along with antibody and protein biomarker data. / Conclusions: The proposed ICON 2022 diagnostic criteria and classification system address the historical lack of consensus in optic neuritis classification. They provide a structured methodology, reducing the risk of misdiagnosis and serving as a foundation for future validation studies.
