Abstract
Internuclear ophthalmoparesis (INO) serves as an essential clinical marker for evaluating brainstem integrity and provides valuable insights into neural repair mechanisms. The future of neurological treatment lies in enhancing brain repair strategies, making it crucial to simplify diagnostic and therapeutic approaches for greater accessibility and effectiveness. INO, characterized by its distinctive eye movement abnormalities, is highly suited for video documentation, enhancing objective assessments and clinical teaching. Anatomically, INO results from a lesion in the medial longitudinal fasciculus (MLF), a single axonal tract vital for coordinating horizontal eye movements. As an outcome measure, INO is both reproducible and exhibits a robust effect size, making it a reliable indicator for assessing treatment efficacy. In clinical trials, INO is especially relevant due to its relatively high prevalence in multiple sclerosis (affecting approximately 25% of MS patients), ensuring sufficient participant availability for research. Understanding and recognizing INO in routine clinical practice have significant treatment implications, underscoring the importance of tailored interventions and comprehensive patient care strategies. Future studies and applications will focus on integrating these insights into broader therapeutic frameworks to improve outcomes in patients with demyelinating diseases and other neurological disorders.
