Abstract

Title: Language impairments in children with Cerebral Palsy (CP): A Scoping Review / Design: Scoping Review / Objective: Little is known about the profile of language impairments in children with CP. This review sought to describe language impairments in children with all sub-classifications of CP. Methods or tools used to assess and measure language performance in children with CP were also reported. / Method: This Scoping review was conducted in accordance with the Joanna Briggs Institute (JBI) methodology for scoping reviews. The Preferred Reporting Items for Systematic Reviews and Meta-Analysis extension for scoping reviews (PRISMA-ScR) was used to guide reporting1. The International Classification of Functioning, Disability & Health (ICF)2 was used as the conceptual framework. Studies that identified language impairments in children with CP (0-18 years) since the 1980s were included. Seven relevant electronic databases and a targeted sample of the grey literature without any language restrictions were searched. Two independent reviewers assessed articles for eligibility and three independent reviewers extracted relevant data. / Results: From initially screening 11,273 studies, 387 full text studies were assessed for eligibility and 65 studies were included. Most studies were conducted in the USA (n=22), used a quantitative design (n=62) and were from journal article sources (n=62). The total number of participants was 2350 ranging in age from 0 yr 8 mths to 18 yrs 2 mths, and sex was reported in 59 studies (Male=1194, Female= 969). Sample sizes were small (n= <55) except for 10 studies. Significant variability was found in how CP was described. Impairments were identified mostly at a BFS-level with receptive language being the language domain most frequently described (n=43). The Peabody Picture Vocabulary Test (PPVT)3 was the most frequently used tool across studies. / Conclusion: Research on language impairments in children with CP has been limited to small heterogenous samples and largely focused on BFS-level (receptive language) outcomes. Future research focussed on language outcomes beyond an impairment level is needed with larger samples of children with CP. Expert consensus is needed to optimise research-reporting to enable better understanding of the breadth of language impairments across CP sub-classifications. / References: 1. Tricco, A.C., et al.
